Please click here to download our Scholarship Application. In order to apply for the David Scholar scholarship, the Foundation must receive the following from each applicant by the April 10th deadline:

  • The completed original application, signed by applicant and his or her parent(s) or legal guardian(s); along with the following attachments:
  • A copy of the first two pages of the most recent federal tax return (i.e., 1040) of applicant’s parent(s) or legal guardian(s) attached as indicated in the application. NOTE: In instances where parents file separate returns, a copy of each parent’s tax return is required. If parents are separated or divorced, a tax return must be submitted for both parent(s) or guardians(s). (The Foundation will accept tax information separately in order to maintain confidentiality of each parent). If parent(s) or guardian(s) are self-employed, a copy of Schedule C, Schedule F, etc., is required with a copy of the tax return;
  • A copy of the FULL Student Aid Report (SAR) from the Free Application for Federal Student Aid (FAFSA) report, showing the Expected Family Contribution (EFC);
  • A copy of the financial aid award letter received from the college being attended in the fall (if this is not available before the application deadline, send the letter separate when the letter becomes available);
  • Two completed teacher evaluations, signed and placed in signed, sealed envelopes and forwarded to the Foundation with the completed application;
  • A copy of applicant’s official high school transcript, showing test scores, placed in a signed, sealed envelope and forwarded to the Foundation with the completed application;
  • Completed personal statement essay questions as requested in the application; and
  • In addition to the original application and attachments, TWO copies of: the completed application, tax return information, Student Aid Report, financial aid award letter, and essays, with each complete set of copies being stapled together (NO binders or notebooks, please).

The deadline for Scholarship Applications is April 10th (for enrollment in the fall semester of the same calendar year).

Each Scholarship Application is reviewed by our Scholarship Committee, which includes at least one member of the David family. The Applications are quantitatively and qualitatively evaluated and the applicant pool is refined to those that best fit our criteria. To reiterate, one of the most highly weighted factors for serious consideration is financial need. All other things being equal, a candidate with greater financial need will generally be awarded a Scholarship over one with less financial need.

Scholarship Conditions

To ensure renewal of a scholarship we require that David Scholars:

  • Maintain a minimum cumulative 2.5 grade point average
  • Complete at least 12 credit hours per quarter or semester
  • Provide us with an official transcript after each quarter or semester
  • Behave appropriately and remain in good standing with the college or university

Frequently Asked Questions (FAQ)

For applicants:
  • Can I apply if I'm already in college?
    No, the Scholarship is only available to high school seniors attending a Stark County high school.
  • Can I apply if I attend a high school outside of Stark County, Ohio?
    No, the Scholarship is only available to seniors at Stark County, Ohio high schools. You do not need to have been born in Stark County, Ohio, but you do need to be graduating from a Stark County, Ohio high school in order to apply for a Scholarship.
  • Can I use the Scholarship for room and board?
    Scholarship funds can only be used for tuition and books.
  • Can I use the Scholarship for Graduate school?
    The Scholarship Program can only be used for undergraduate or other post-secondary education.
  • Do I have to attend an Ohio college?
    No, you can attend any accredited college or university in the continental United States. List of Universities.
  • Why do you need my parent's tax information?
    Our Scholarships are based on financial need. Tax information for your parents helps us to determine the most qualified applicants relative to this measure.
  • Can I use the Scholarship for a trade school?
    Yes, Scholarship funds can be used for trade school, as long as the school is accredited.
  • Can I apply for a Scholarship if I attended a private high school?
    Scholarships are available for state accredited private high schools in Stark County, Ohio.
  • Can I apply if I am home schooled?
    No, the Scholarship is only available for students attending a Stark County, state accredited high school.

  • What is the Scholarship deadline?
    All Scholarship applications must be postmarked by April 10th for consideration for the fall semester of the same calendar year.
  • When are Scholarship awards announced?
    Scholarship awards will be announced on our website by June 1st.
  • Where do I get an application?
    Applications can be obtained at your high school guidance office, or at our website. To download a scholarship application please click here.
  • Can I apply if my sibling(s) have already received a Scholarship?
    Yes, siblings of previous Scholarship recipients can apply.

For Scholarship recipients:

  • What happens if tuition at the school I plan to attend is less than the amount of the Scholarship?
    If tuition and books at the school you are attending are less than the scholarship disbursement, you must provide a copy of the tuition and books invoice to the Foundation. Excess amounts cannot be used for room or board.
  • How will the funds be disbursed?
    Checks will be issued by quarter or semester, depending on the college that you attend, and will be made payable directly to the college or university.
  • What if I fail to maintain the requirements of the scholarship?
    The first time you fail to meet the scholarship’s requirements, your scholarship check will still be issued to the school you are attending. If you fail to achieve any requirements for a second grading period, you will no longer be eligible to receive scholarship funds at any time in the future. Regardless, you must still send us an official grade transcript after each semester/quarter.
  • What do I need to do to renew the Scholarship?
    In order to renew your Scholarship, an official transcript must be sent to the Foundation’s office, showing an accumulative GPA of 2.5 or higher and a minimum of 12 credit hours for each semester/quarter. Lastly, we expect our Scholars to be model citizens in the community. Therefore, any illegal act resulting in a conviction, suspensions from school due to violations of school policy or any other acts deemed unethical or immoral will result in forfeiture of your Scholarship.
